Request for a Support Animal

The questions below, along with information in the provider form, are designed to give SUNY Poly a more complete picture of your needs and the appropriateness of the requested accommodation. There are no right or wrong answers, please fill them out to the best of your ability.

What is your understanding of the purpose of a support animal?

Why are you requesting a support animal at this time?

How do you think having the support animal will help you in dealing with your disability?

What is the style of on-campus housing you have been assigned to or are requesting?

How will the animal provide you with access to residence hall living not otherwise possible? In other words, what can you now do/participate in because a barrier has been lessened or eliminated due to having a support animal?

What is the type, size, and care of needs of the support animal?

Is the animal housebroken/trained to relieve itself in designated areas as appropriate for the type of animal?

What is the size of cage/crate needed for containment?

As noted in our policy, the support animal must be caged or crated when you are not in your room. Are you concerned at all about leaving your support animal alone in a crate for potentially long periods of time?

How will you properly control and care for the animal, along with a specified plan for doing so?

Leaving the animal in someone else's care is not permitted. If you leave campus for a night or weekend, you must either take the animal with you or find some place, off campus, for the animal to stay. Have you identified someone who can take the animal for you in the event that you are not on campus overnight (you will need to identify an alternate caregiver by name later in the process)?

Please note: Some websites sell certificates, registrations, and licensing documents for assistance animals to anyone who answers certain questions or participates in a short interview and pays a fee. Certificates, registrations, and licensing documents for an emotional support animal does not exist and therefore SUNY Polytechnic Institute will not consider such documentation as reliable. Under the Fair Housing Act, a housing provider (SUNY Polytechnic) may request reliable documentation when an individual requesting a reasonable accommodation has a disability and disability-related need for an accommodation that are not obvious or otherwise known. In United States Department of Housing and Urban Development’s (HUD’s) experience, such documentation from the internet is not, by itself, sufficient to reliably establish that an individual has a non-observable disability or disability-related need for an assistance animal. SAS is a National Voter Registration site and we are required to provide an opportunity for you to register. Applying to register or declining to register to vote will not affect the amount of assistance you receive at SUNY Poly or from SAS.
