The questions below, along with information in the provider form, are designed to give SUNY Poly a more complete picture of your needs and the appropriateness of the requested accommodation. There are no right or wrong answers, please fill them out to the best of your ability.
Please note: Some websites sell certificates, registrations, and licensing documents for assistance animals to anyone who answers certain questions or participates in a short interview and pays a fee. Certificates, registrations, and licensing documents for an emotional support animal does not exist and therefore SUNY Polytechnic Institute will not consider such documentation as reliable. Under the Fair Housing Act, a housing provider (SUNY Polytechnic) may request reliable documentation when an individual requesting a reasonable accommodation has a disability and disability-related need for an accommodation that are not obvious or otherwise known. In United States Department of Housing and Urban Development’s (HUD’s) experience, such documentation from the internet is not, by itself, sufficient to reliably establish that an individual has a non-observable disability or disability-related need for an assistance animal.
